This Application Form is not intended as a formal requirement but is arranged to provide convenience for your valuable applications. Therefore, you can submit your request, which includes your explanations regarding the right you wish to exercise among the rights specified in Article 11 of the Law, to our company by filling out the application form below or with an alternative text prepared by you (which must contain the mandatory elements required by the Communiqu\u00e9 on Application Procedures to the Data Controller). You can deliver a signed copy to the address <em>\"Ahievran Cad.
