This Application Form has been prepared by Tema İş Makinaları Servis Ltd. Şti., in its capacity as the data controller, to evaluate applications made in accordance with the relevant provisions of the Personal Data Protection Law No. 6698 and to respond to you promptly within the legal period. This Application Form is not intended as a formal requirement but is arranged to provide convenience for your valuable applications. Therefore, you can submit your request, which includes your explanations regarding the right you wish to exercise among the rights specified in Article 11 of the Law, to our company by filling out the application form below or with an alternative text prepared by you (which must contain the mandatory elements required by the Communiqué on Application Procedures to the Data Controller). You can deliver a signed copy to the address "Ahievran Cad. No:99 Ostim, Yenimahalle/ANKARA" in person, through a notary, or by other methods specified in the Law.

If the applicant is a third party other than the Data Subject, documents justifying the right to make this application on behalf of the data subject (such as a Power of Attorney, guardianship/custody relationship) must be sent attached to the said applications.

Your application, submitted to Tema İş Makinaları Servis Ltd. Şti. by filling out and signing the Application Form above, will be concluded free of charge as soon as possible and within thirty days at the latest from the date your request reaches us, in accordance with Article 13/2 of the Law, depending on the nature of the request. However, if the transaction requires an additional cost, the fee in the tariff determined by the Personal Data Protection Board may be charged.

Our company does not accept liability for claims arising from applications based on incorrect or outdated information and documents provided by you in this "Application Form to the Data Controller" or applications made through an unauthorized representative.
